I've drawn up a feedback form to send to the folks who attended the seminar. Having never needed to do something like this before, I figured I'd dump it on ask you! Anyone have any suggestions?


Thank you for attending the seminar held at your school!

I hope that you enjoyed it as much as I did. I am always looking for ways to improve my seminars, and the best way that I have found to do that is to ask for feedback from attendees. To that end, I would like to ask you to take a few minutes and fill out this brief feedback form and return it to your instructor.

Your comments and opinions are important to me. Please help me make future seminars better!

This looks clear and do-able. Other possible items might relate to how clear the objectives were (either in the pre-seminar (advertizing) material or in the seminar itself, or both), and how well the seminar met those objectives.

You might also ask about what motivations people had in taking the seminar -- e.g., "the blurb made it sound intriguing", "personal recommendation," "desire to be more effective in a confrontation", etc, and if those desires/expectations were met.

I'd add something about the objectives -- whether they were met or not.

I'd add as a follow-up to #9: what did you want to see less of? and give choices.

Other than that, it looks OK.

For the pre-seminar advertising, that is almost always handled by the host, not the presenter. And just between you and me (and the rest of the folks who read this LJ ::G::) what the host put on the poster in his school didn't quite match up with what I'd offered. Fortunately, I found out about this far enough in advance to be able to tailor the info to what he'd advertised!
