I've drawn up a feedback form to send to the folks who attended the seminar. Having never needed to do something like this before, I figured I'd dump it on ask you! Anyone have any suggestions?


Thank you for attending the seminar held at your school!

I hope that you enjoyed it as much as I did. I am always looking for ways to improve my seminars, and the best way that I have found to do that is to ask for feedback from attendees. To that end, I would like to ask you to take a few minutes and fill out this brief feedback form and return it to your instructor.
