Okay, I'm getting ready to do another martial arts seminar. What additions/changes would you recommend for the following feedback form?

For the first item, you could add a response to cover, "It wasn't what I was expecting, but this isn't a dissappointment, just different."

I might be curious as to what people find useful (rather than their favorite part). Some things can be fun without being the most useful, and visa versa. I would also be curious as to what people might hope to gain from this information, and what they would hope to learn next. These could be write-ins.

'What made you decide to come on the course?'

I guess it depends on what info are you wanting to get *out* of the form. I am assuming you want to improve things so more people come to the next one. So why they came, would they recommend it, would they come again are the main points.
